Application for Haemodialysis Programme
Who is eligible?
- Applicants applying for the Haemodialysis Programme at Tzu Chi Renal Dialysis Centre must meet the following eligibility criteria.
- Applicants whom are unable to the meet the eligibility criteria will be considered on a case-by-case basis. Please kindly contact 63780388 before submission.
Social Admission Criteria
- Subsidised patient must be referred by Medical Social Workers or Renal Coordinators
- Subsidised patient should be eligible for Means Test Tier 1-5 of the Community Haemodialysis Subsidy Framework.
- Subsidised patient must not own a private property with annual value of more than $21,000 per annum.
- Subsidised patient must agree for home visit assessment & provide necessary supporting documents.
- Patient must be 18 years of age or older.
- Patient must not be a Civil Service Card (CSC) Holder.
Medical Admission Criteria
- Hemodynamically stable and suitable for outpatient haemodialysis placement.
- Patient must not have persistent intra-dialytic hypotension as defined by symptomatic drop in systolic bold pressure of > 30mmHg in the last 3 consecutive haemodialysis sessions.*
- Stable and asymptomatic Ischemic Heart Disease or Congestive Cardiac Failure.
- Patient with long term oxygen need will only be admitted if they are using own oxygen concentrator. Oxygen cylinder will not be accepted due to fire hazard.
- Ongoing active infection that require air-borne precaution/isolation will not be admitted. Other infection precaution will be admitted based on centre capability/case by case basis.
- Patient must not be behaviourally disturbing and require more than minimal restraining.
- Patient must be able to tolerate at least a recline position of 30 degree for 4 hours.
- Patient must be Hepatitis B negative.
- Tzu Chi Renal Dialysis Centre is not a MOH designated High Dependency Haemodialysis (HDHD) Centre.
How is the application process like?
Documents Checklist
Once the application and complete set of documents are received, the intake officer and medical director will review them for admission eligibility. The referring party will be notified of the outcome.

- Documents on financial assistance received from other organizations (If Any)
*Patients applying for financial assistance and/or dialysis transport assistance must submit documents 2–11. For documents 3 and 4, please also provide those of family members residing at the same registered address.
